After you have swallowed the water with the infected fleas, the small immature forms called the larvae are released, move and mature in the wall of the intestine, from which they move to the skin and produce ulcers through which the female now discharges new immature forms after contact with water. The ulcer can cause the symptoms of intense itching and a burning feeling which can be disabling. The head of the female is often seen at the base of this ulcer.
