Smallpox is caused by the variola virus and is most often spread in air droplets when an infected person coughs, sneezes or talks. In rare cases, the virus could also be spread through other body fluids and contaminated clothing or beddings. Smallpox is highly contagious and fatal. People who have it are most infectious during the first week of illness.
