When too much fluid collects in that pouch around the heart, it affects the way the heart fills with blood when carrying out its pumping function. This can lead to blood pressure dropping so low that the patient could die. Also if the irritation around the heart continues for a long time, the heart loses its elasticity and doesn't beat well.
