Usually, it is better if you have acute OIs to begin HIV treatment at once especially if the infection is difficult to treat. However, sometimes it is better to delay beginning HIV treatment and instead only receive treatment for the opportunistic infection, esp if there are concerns about drug interactions or overlapping drug toxicities. Discuss with your doctor for treatment options.
