Vaccination - Records
A vaccination health record helps you and your health care provider to keep your child's vaccinations on schedule. The record should be kept from the time your child receives the first vaccination, and it's important that you carry this record with you each time you take your child for a new vaccination to make sure all the vaccines have been recorded.
